Welcome to the Winter 2024 Edition of Stroke Insights and our very first video of this new quarterly series!

Our video presentation titled: Stroke Insights – Determining the Date/Time of Last Known Well (LKW)

This presentation begins with a review of the data element “Date and Time Patient Last Known Well”, and then continues to explore the difference between Get With The Guidelines (GWTG) ® Last Known Well and The Joint Commission (TJC) Last Known Well. The following topics will be explored:

  • Decipher the earliest most appropriate date/time LKW
  • Using the GWTG® hierarchy
  • Identifying LKW based on transient symptoms

Each of the covered topics will include specific abstraction guidance that is defined in the current Specifications Manual and Coding Instructions. We will review the application of these guidelines using examples and rationales.

This presentation is based on the information included in the most current version of The Joint Commission Specifications Manual version 2023B (Discharges July 1, 2023 through December 31, 2023) and the June 2023 GWTG®Stroke Coding Instructions. For the most accurate guidance, please refer to the versions of the Specifications Manual and Coding Instructions that correspond to the Discharge Date(s) being reviewed.
