In this month’s session of the Oncology Coding Break, we will delve into the complex anatomy of the liver, bile ducts, and adjacent structures, exploring both their physical relationships and physiological functions. A clear understanding of this anatomy is essential for accurate cancer coding, particularly when assigning primary site and histology codes.
We will examine the fundamental differences between the liver’s hepatocytes and the bile ducts’ cholangiocytes, and how these cellular distinctions influence the classification of liver and bile duct malignancies. Special attention will be given to coding the correct primary site for histologies specific to each cell type.
Additionally, we will take a focused look at ICD-O topography code C24.0, which includes the cystic duct, perihilar bile ducts, and distal bile ducts. We’ll explore how to differentiate these anatomical sub-sites using a schema discriminator in our coding software, ensuring accurate site-specific classification. This detailed review will enhance both anatomical understanding and coding precision for malignancies arising in this complex region.
